A fee of $10.00 will be charged for an official copy of your immunization records.

  • To Receive Your Immunization Records By Mail
    Send a signed request with name, I.D# and address along with a $10.00 check made out to Dowling College directed to the Registrar Office.
     
     
  • To Pick Up Your Immunization Records In Person
    Go to the Registrars Office with $10.00 cash or check.
